Gentiva Hospice offers care to patients with a life-limiting illness. As a guide, people often enter hospice care when they’re diagnosed with a life-limiting condition and curative treatments are either no longer desired, effective, or create side effects that prolong discomfort or pain. Because hospice is based on a personalized care plan and not a specific location, patients can choose to receive hospice care in the familiar surroundings of their own home. They can also choose to receive hospice care at a relative or a friend’s home, an assisted living facility or a nursing home. With our office located in Hermantown, Mn. we service roughly Greater Duluth, over into Wisconsin and south towards Moose Lake, MN. Our care team is made up of physicians, nurses, aides, social work, chaplaincy and volunteers to help support individuals and their families as they navigate this new and final season of life.
